Photography/Audio/Video Consent and Release Form

Grant to Chelsea the absolute and irrevocable right and permission to use, publish and copyright such images and recordings.

Grant to Chelsea the right to use, re-use and publish in whole or in part, any reproductions of such images and recordings, in any and all media of corporate communications, public relations, advertising or promotion, without compensation to me or my business.  These uses include, but are not limited to, illustrations, bulletins, exhibitions, videotapes, reprints, reproductions, publications, advertisements, and any promotional, internal or external materials and/or communications in any medium now known or later developed, including the Internet.

Agree that such photographs/recordings are the exclusive property of Chelsea, and Chelsea may use the photographs/recordings in any manner.

Release, defend and hold Chelsea and its agents and assigns harmless from any and all third party claims which may arise out of or are in any way connected with such use.

Agree that neither Chelsea nor anyone authorized by it to use such photographs/recordings will have any liability for any special or illusionary effect or distortion that may occur in publication or reproduction of such photographs/recordings or likeness.

Release and discharge Chelsea from any and all claims and demands arising out of, or in connection with, the use of the photographs/recordings, including any and all claims for libel.

Waive any right to inspect or approve the finished photographs/recordings and/or other materials that may be used in conjunction with such photographs/recordings now or in the future, whether that use is known or unknown to me.

Have read and understood these terms.

Consent to Chelsea’s use of such photographs, recordings, my voice and the name and likeness of myself and my business, if applicable, to promote Chelsea, and/or its affiliates and their activities.

Warrant to Chelsea that I am of legal age to sign this document, I am or my business, if applicable, is the owner or lessee of the premises and/or the equipment which is the subject of the application of the Chelsea product to which this document relates and am authorized to sign it on behalf of my business, if applicable.
